Please be sure to communicate with your referring doctor’s office about whether your insurance requires a referral and authorization is needed to see a specialist and whether an authorized visit has been approved.

• If you have been referred for a nerve conduction and electromyogram (NCS/EMG), please do not apply any oils or lotions to the limb that will be evaluated on the date of your testing.

• If you have been referred for EEG, please be sure to shampoo your hair and scalp on the date of the procedure.

• We welcome family members and friends with the approval of the patient. Family and friends can often provide helpful information based on their observations. Partners, friends and family members are encouraged come to the appointment if you are comfortable with them being present with you.
